IĮ “Periodont” (hereinafter referred to as the Clinic) is committed to protecting the privacy of its patients. This privacy policy provides information on how we process patients’ personal data.

GENERAL PROVISIONS

  • The Clinic is the controller of your personal data.

PATIENT PERSONAL DATA PROCESSED BY THE CLINIC

  • When providing dental services, the Clinic processes the following patient personal data: first name, last name, home address, date of birth, personal identification number, identity document details, email address, and phone number. When providing dental services, the Clinic also processes patients’ health data: records of the patient’s visits to the Clinic, data regarding the patient’s health condition, diagnosis, diagnostic, treatment, and care measures applied, and temporary incapacity for work.
Purposes and Legal Basis for the Processing of Personal Data
  • The personal data listed above is necessary for the identification of the individual and the provision of dental services, and the Clinic is required to process it under the laws of the Republic of Lithuania. Dental services cannot be provided without this data.
  • The personal data listed above may be processed for the purpose of debt management if the patient fails to settle their account with the Clinic within the timeframes specified in the contract. The General Data Protection Regulation grants the right to process personal data if such processing is necessary to pursue the legitimate interests of the data controller and/or to establish, exercise, or defend legal claims.
  • The patient’s personal data—first name, last name, email address, and phone number—is processed for the purpose of providing information about services, provided that written consent from the patient for such data processing has been obtained.
Retention period for personal data
  • Patients’ personal data is processed for the purpose of providing dental services and is retained for the period specified by law (15 years), with the exception of patient registration data for the provision of dental services, which is retained for 3 (three) years.
  • Patients’ personal data processed for the purpose of debt management is stored for as long as the legal dispute with the patient lasts.
  • Patient personal data processed for the purpose of providing information about services is stored for two years from the date of withdrawal of consent or from the date of the Clinic’s decision to no longer process personal data for the purpose of providing information about services.
Provision of personal data
  • Patients’ personal data is processed for the purpose of providing dental services and is disclosed to third parties only with the patient’s written consent, except in cases provided for by law. Patients’ personal data may be disclosed without the patient’s written consent to healthcare institutions where the patient is or was treated or cared for, and to competent authorities in accordance with the Law on Patients’ Rights and Compensation for Damage to Health of the Republic of Lithuania and other laws of the Republic of Lithuania.
  • When providing dental prosthetics or orthodontic treatment services, the patient’s personal data (first name, last name, and health data necessary for the manufacture of dental prostheses and/or other medical devices) is provided to the dental laboratory where the patient’s dental prostheses and/or other medical devices are manufactured. If the patient refuses to provide personal data to the dental laboratory, the necessary dental prostheses and/or other medical devices cannot be manufactured, and the Clinic will be unable to provide the patient with the necessary dental services.
  • When providing dental services covered by the Compulsory Health Insurance Fund (dental prosthetics and dental sealants), the patient’s personal data (first name, last name, personal identification number, description of dental services provided) are submitted to the territorial health insurance funds.
  • When providing dental services covered by voluntary (supplementary) health insurance, the patient’s personal data (first name, last name, personal identification number) and health data (description of the healthcare services provided) are provided to the insurance company with which the patient has voluntary (supplementary) health insurance, subject to the patient’s written consent.
  • Patients’ personal data processed for debt management purposes may be provided to attorneys, debt management and collection agencies, bailiffs, courts, and other entities involved in debt collection, but only to the extent necessary for debt collection.
  • Patients’ personal data processed for the purpose of providing information about services is not disclosed to any recipients.
  • When providing dental services, the Clinic processes patients’ image data obtained from the Clinic’s cameras. Image data is obtained when doctors photograph the patient before, during, and after scheduled treatment and is intended for monitoring the treatment and/or healing process. Patients are photographed only with their written consent.

Purpose and Legal Basis for Data Use

  • Patient image data obtained from the Clinic’s cameras is processed solely for the purpose of providing dental services. The General Data Protection Regulation grants the right to process such data if it is necessary for the provision of healthcare services. The patient has the right, by separate written consent, to allow the Clinic to process de-identified (anonymous) image data for scientific or marketing purposes.
Retention Period for Personal Data
  • Patient image data processed for the purpose of providing dental services is treated as medical record data and is retained for the period established by law for the retention of medical records (15 years).
Disclosure of personal data
  • Patient image data processed for the purpose of providing dental services is disclosed to third parties only with the patient’s written consent, except in cases provided for by law.

PERSONAL DATA SECURITY

  • Patients’ personal data is processed in accordance with appropriate organizational and technical security measures designed to protect personal data against accidental or unlawful destruction, alteration, disclosure, or any other unlawful processing.

PATIENT RIGHTS

  • Patients have the right to request that the Clinic allow them to access their personal data free of charge, and to have it corrected or deleted, or to restrict the processing of the data, or to object to the processing of the data. These patient rights are exercised in the cases and in accordance with the procedures established by the General Data Protection Regulation and other legislation.
  • Patients have the right to withdraw their consent to the processing of personal data, if such consent has been given. Patients may withdraw their consent via email or by any other means convenient to them.
  • Patients who believe that their rights regarding the processing of personal data have been violated have the right to file a complaint with the State Data Protection Inspectorate.
